Important medical disclaimer

Educational tools only — not medical care

Migraine Trail is not a healthcare provider, medical service, or medical device. These tools use information you enter and simplified educational rules. They may be incomplete or wrong and cannot examine you.

  • Results do not diagnose, rule out, confirm, or predict any disease, headache type, trigger, emergency, or personal medical risk.
  • Do not start, stop, combine, delay, or change a medicine or treatment—and do not delay professional care—because of a result.
  • Labels such as “pattern,” “alignment,” “score,” “threshold,” “risk,” or “urgency” are educational summaries, not clinical findings or probabilities.

Discuss symptoms and decisions with a licensed healthcare professional who knows your history. For a child, involve a parent or guardian and an appropriate pediatric clinician.

What to record

Keep it simple enough to complete every day

Mark headache and migraine days, then add severity, duration, symptoms, medication response, and anything that may have contributed. A simple, consistent record is more useful than a detailed diary you stop using.

  • Headache and migraine days
  • Pain severity and duration
  • Aura and other symptoms
  • Medication and dose
  • How much relief you got
  • Weather, sleep, stress, or cycle notes

Why keep a headache diary?

A diary gives you and your clinician a clearer view of attack frequency, symptoms, treatment response, and possible patterns. The Migraine Trust recommends recording basics such as date, duration, severity, symptoms, medication, and potential triggers.

Prefer automatic tracking?

Log attacks by voice, see patterns over time, and create doctor-ready reports with the Migraine Trail app.

Printable migraine diary questions

How long should I keep a migraine diary?

One month is a useful starting point. Several months make it easier to see changing patterns, treatment response, or a possible relationship with your menstrual cycle.

Can I save the diary as a PDF?

Yes. Select “Print or Save PDF,” then choose your browser’s “Save as PDF” destination instead of a physical printer.

Does this diary diagnose migraine?

No. It organizes information for your own review and for conversations with a qualified healthcare professional. It is not a diagnostic or emergency-assessment tool.
